To Obtain Credit or Confirm Credit

• Request for Unpaid Credits not Reported by Provider – to report course attendance. DO NOT use the Affidavit of Compliance form below to report attendance.
• • Application for Accreditation – For attorneys to seek accreditation for a course not accredited by the provider.
• Affidavit of Compliance – Only applies to attorneys who receive April 30 Notice of Non-Compliance or are on the draft suspension order issued in June each year. Use this form to send course information to avoid suspension or to request reinstatement.  This form may not be used to report attendance.

• Request for Indigent Representation Credits
• If you are seeking credit for college courses taught under Tenn. Sup. Ct. R. 21 § 4.03 (b), use the Request for Unpaid Credits form and include a letter from the college indicating the name of the course, how many credit hours students received for the course, and the last date the course met.

Requests are heard by the Commission on the third Tuesday of each month. Requests must be received 10 days prior to the hearing, or the request will be heard at the next regular Commission meeting.
